- And click on the Members Login
- A Login Page will open, Click on Not a member “Join or Renew“.
- Select “New Members” from the list of Applications Forms.
- Now, Select “Membership Application Form“.
- A form will be open, Fill up the form and submit it.
- Once you have registered, you will be able to access your account information, including your username and password. You can also view your medical record online, print or download it, and share it with family and friends.
